Please Note:

Online forms MUST BE completed on a desktop or a laptop. Phones and tablets will not let you submit the forms once you have completed it (frustrating… we know…)

If you do not have access to a desktop or laptop at home, we do have paper copies of the intake forms available to be filled out in the office. You will need to arrive 15 to 20 minutes before your appointment time to fill them out. If the forms are not completed before your appointment time, we may have to reschedule your appointment.
